Eligibility

Palliative Care

  • Patients with an advanced, serious life-limiting illness, such as Cancer, COPD, Dementia, CHF or Failure to Thrive
  • Patient chooses to continue curative treatment

Hospice Care

  • Six months life expectancy
  • Patient chooses to forego curative treatment

Hours Available

Palliative Care

  • Monday-Friday, 9am - 5pm
  • Phone support 24 hours a day

Hospice Care

  • 24 hours a day

Financial Responsibility

Palliative Care

  • Medicare, Medi-Cal and private insurance billed for MD/NP visits (patients may have co-pays); patient billed a monthly, out-of-pocket fee, which covers all other services
  • Private insurance may include all services
  • Financial assistance is available for those who qualify

Hospice Care

  • Medicare and Medi-Cal, which cover 100% of services
  • Private insurance, which may have co-pays
  • Financial assistance is available for those who qualify

Medications, DME and Medical Supplies

  • Not covered by Palliative Care

Hospice Care

  • Covered, if related to the hospice diagnoses

Primary Provider

Palliative Care

  • Patient have their own Primary Care Provider (PCP) to work with the palliative care team

Hospice Care

  • Patient may choose their own PCP or our hospice MD or NP to work with the hospice team
