What is Palliative Care?
Palliative Care focuses on the relief of symptoms and stress related to a serious illness. People with serious illnesses have specialized needs and often fall between traditional home health services and hospice care. Palliative Care gives these individuals much needed assistance in navigating the complicated health care system.
Hospice East Bay’s Palliative Care program works within a team-based model to coordinate care with your other health service providers, such as your primary care physician (PCP) and other specialists. We will work with you to develop a plan for care that is aligned with your needs and wishes, and which involves all of your doctors and services.

We can help with:
- Filling out complicated medical forms;
- Managing medications and prescriptions;
- Educating family members and caregivers on how to give medications and use medical equipment;
- Organizing care conferences to help resolve family conflicts;
- Arranging limited volunteer services for short-term needs;
- Assisting with difficult emotional and logistical transitions;
- Coordinating care with your doctors and other programs.
- All insurances will be verified for coverage prior to enrollment
- All services covered for most patients with Blue Shield, Canopy Health and Health Net; spiritual care and volunteer services for limited, short-term needs are available at an additional cost for other insurances.
- Medicare Part B insurance pays for 80% of MD/NP visits with 20% co-pay which may be covered by secondary insurance.
- Some insurance covers additional RN, MSW services.
- Financial assistance is available for those who qualify.
Palliative Care of the East Bay accepts patients who:
- Are 18 years of age or older;
- Have a diagnosis of a serious illness such as cancer, COPD, dementia, or failure to thrive;
- Have a primary care provider;
- Are not currently receiving hospice services.
- Patient may choose to continue with curative care.

As the leading national accreditation agency for hospice and home health organizations, CHAP’s certification ensures that our quality of care meets the industry’s highest nationally recognized standards in palliative care.
- Monday-Friday, 9am – 5pm
- Phone support 24 hours a day
- Billed to insurance, would be similar to the orders submitted by one’s PCP or specialists.
- MD, NP, RN, MSW, Spiritual Care Counselor (SCC), limited, short-term volunteer visits at your residence.

Our Palliative Care Team
Practitioners
Our Physician or Nurse Practitioner visits your home to learn about your needs and wishes, and develops a plan of care that emphasizes what is most important for you to have the best possible quality of life.
Social Workers
Our Medical Social Worker develops a road map to help you and your family make the right decisions throughout your care.
Registered Nurses
Our Registered Nurse Case Manager coordinates your care and provides information about how your disease may affect you and your quality of life.
Counselors
Our team also includes a Spiritual Care Counselor, available as needed, and our Palliative Care Coordinator, who is available 9am-5pm, Monday-Friday, to answer your questions.
